An attendance certificate will be issued to those who have joined the Endourology Made E-C: EAU-CUHK workshop. All participants are required to stay for the entire workshop for obtaining an attendance certificate.

Personal information: During the registration process, your personal information listed in this registration form will be collected by the organizer(s) and the secretariat (MISSC) for event updates and post-event surveys. The organizer(s) and the secretariat may also use the collected information to update you future conferences, seminars, workshops and other educational activities of these parties via email.

Liability: The organizer(s) and the secretariat accept no liability for personal injuries sustained by, or for loss or damage of property belonging to event participants, either during, or as a result of the event, or during all event activities. It is therefore recommended that participants arrange their own personal health, accident and travel insurance (if applicable).

Personal insurance: The organizer(s) and the secretariat cannot be responsible for health care, dental and ambulance services during their seminars, workshops, congresses, conferences and / or events. Participants are strongly encouraged to enroll in comprehensive medical and travel insurance (if applicable), which should cover the possibility of flight cancellation (if applicable) due to strikes and other causes. The organizers and the secretariat accept no responsibility of any nature whatsoever for personal injury, death, loss or damage to property or participants however caused or arising.
